Support at home is a community-based support programme funded by local authorities that helps older people to tackle issues such as rent arrears, threatened eviction, financial difficulties, social isolation and health problems.
These vital wellbeing services help individual older people to remain in their own homes. Many older people need a small amount of help, advice and sign posting to remain at home, our team make an assessment of each person's individual needs and the suitability of their home, agree a plan of action and work through this with the customer, maximising independence and preventing delaying or reducing the need to mover to higher dependency services.
Who can use these services?
Older adults living in Hammersmith and Fulham and Harrow can be referred to our support at home services. Referrals come from a variety of places, including local housing departments, social services and voluntary groups.
